How to File a Claim?

There are three convenient ways to file a Supplemental Health claim: by phone, on paper, or online at MyUHCFP.com.

1.   To file a claim by phone, simply call (888) 299-2070 and speak with a Financial Protection Member Service Advocate.

2.   To file a claim by paper, please use the appropriate claim form provided below. The instructions for submitting a paper claim are located at the top of each form.

3.   To file and track Supplemental Health claims online (excluding Wellness Benefit claims), go to MyUHCFP.com.
